Critical Illness insurance pays a lump-sum benefit directly to you if you have a new diagnosis of a covered condition. This could include illnesses such as cancer or health events such as a heart attack or stroke. You can use that benefit payment as you see fit.

Your Coverage Options

You must elect coverage for yourself to elect coverage for dependents such as your spouse or children. Spouse and child coverage cannot exceed 50% of your coverage amount.

1 Your coverage is guaranteed if you are actively at work. Your dependents’ coverage is guaranteed provided they are not subject to medical restrictions as set forth on the enrollment form and in the plan’s Certificate. Some states require the insured to have medical coverage. MetLife will not pay a benefit for a Covered Condition that is diagnosed prior to the coverage effective date.

*By law, if a domestic partner does not qualify as a tax dependent, the cost for their benefits cannot be paid pre-tax, and the “value” of associate and employer-provided domestic partner contributions is taxable.

Health Screening Benefit

Receive a $75 benefit per covered person per calendar year for completing a covered health screening test (such as a physical exam, total cholesterol blood test, mammogram, lipid panel, and more).

This benefit is available even if the screening is covered at 100% as preventive care. To receive the benefit, you must submit proof of completion, regardless of whether you are enrolled in the Neighborly Medical/Rx plan.

What You’ll Pay for Coverage

Your premium contributions depend on your age and whether you use tobacco. There is no additional premium coverage for child coverage. Sample rates for the $5,000 and $10,000 benefit are shown below.
